The basic way you do this is to cut a hole behind the TV near where the AV connectors are. You also cut a hole directly below that hole behind your equipment cabinet. Both of these holes should be in a spot that they will be hidden by the TV and equipment cabinet when you are done. The two holes need to be lined up vertically. You then need to drop the cables from the top hole and fish them out from the bottom hole or get a "fish tape" at a hardware store. It is also a good idea to put junction boxes inside the holes and use a coverplate to make things look better.

Mounting a flat screen on the wall does look nice but just putting it on a TV stand is much easier.

The biggest mistake people make is mounting the TV too high on the wall. It should be at a height so that you can look directly at it without looking up too much when you are seated. Above a fireplace is way too high.

You can cut a hole in the wall, it all depends on where your TV socket and power supply is, I would go down to your local DIY shop and ask.

Hiding the cables, either use conduit or dig out the wall, bury the cables and plaster over and then re-paint or paper.
